Man-year

These data are intended to encompass all kinds of activities within home care services and institutions, and are not just limited to health care functions. The data also contains administrative and management staff and, with regard to institutions, kitchen and cleaning functions, etc. Staff are registered on the basis of their formal education as of December 31. Figures are expressed in man-years, which means that the total number of part-time employees is converted into the number of full-time employees. Thus, the total number of man-years is based on the assumption of staff stability over the year. Employees without the specified training are registered in collective groups.

Man-years – positions

For staff, the unit of record is the number of hours per week and applies to the hours set for those in permanent full-time or part-time positions. The number of hours per week is recalculated in man-years (full-time positions) by dividing the number of contractual working hours per week. In practice, this means that we have divided by 36 hours for physiotherapists and 37.5 hours for doctors, and 35.5 hours for other professional groups.

Organization

The organization designates the forms of association that individual doctors and physiotherapists have with the municipality. The municipalities offer practitioners four possible forms of association. The two most common forms are the fixed salary and the contract.

Fixed salary doctors and physiotherapists

Fixed-salary doctors and physiotherapists are municipal employees, occupying municipal doctor or physiotherapist positions announced by the municipality. These positions can be full time or part time. Municipalities cover all post office expenses, but the national insurance scheme provides fixed grants to municipalities per post. The duties of these fixed-salary physicians and physiotherapists are set by municipal instructions.

Doctors and physiotherapists under municipal contract

Doctors and physiotherapists under municipal contract are essentially physicians and physiotherapists in private practice who sign an agreement with the municipality or the district. The provisions of this agreement are based on an agreement between the Norwegian Association of Local Authorities, the Ministry of Health and Care Services and the organizations of doctors and physiotherapists. The agreement implies that all doctors are obligated to participate in on-call medical service and municipal responsibilities such as elderly care and preventive medicine. The amount at which the individual doctor must perform these functions and the opening hours of the curative practice are defined in the specific agreements between the individual practitioner and the municipality / district. The income base of doctors and physiotherapists with municipal agreements is made up of reimbursements from National Insurance, patient co-payments and a fixed amount (operating subsidy) from the municipality in compensation for the agreement. Doctors and physiotherapists who combine part-time fixed salary positions and part-time contractual practice are considered to be practitioners with a municipal contract.

Doctors and physiotherapists without municipal contract

There are also practitioners working in private practice who do not have a contract with the municipality. For the individual patient, it does not matter financially whether you visit a compulsory practitioner, a doctor with a fixed salary or a doctor with a municipal contract. Physicians without a municipal contract may, with certain limitations, receive a co-payment as high as the patient is willing to pay.

Following the modifications of articles 5-4, 5-7 and 5-8 of the law of July 1, 1998 on national insurance, reimbursements from national insurance are no longer paid to health professionals working in private practice who do not have no operating agreement with the municipality or county. Exceptions have been made for general practitioners pending the establishment of the ordinary general practitioner regime.

Vacant authorized positions / positions

Authorized positions or fixed-salary positions vacant on December 31 due to absence, illness or other reasons are considered filled if the absence is less than four months, otherwise it is considered vacant.

Over time

Since the statistical year 1994, data on man-years worked by doctors also include hours worked under agreements on extended working time in accordance with the special agreement of the central trade union federation between the Association Norwegian local authorities and the Norwegian Medical Association. This agreement allows, for a maximum period of one year, the signing of an agreement on the extension of working hours beyond 37.5 hours per week. Overtime beyond that is not included.

Home service users

The figures cover all home help and home nursing service users or both who were registered as active users at the end of the year. In general, each person receiving services is considered a user, with one exception; households of two or more people receiving assistance are considered a single user.

Establishments

These are defined as a set of buildings where part of the floor is shared between the residents, the household is shared and the nursing staff is present 24 hours a day. This definition encompasses all kinds of institutions falling under the above-mentioned law and are as follows: old people’s homes, old people’s homes, combined rest and old people’s homes and in addition various municipal housing with full-time services.

Housing for the elderly and disabled

This category includes other types of municipal housing reserved for people in need of assistance due to old age or a disability. They are also offered to people with mental disabilities who have left traditional institutions according to a major reform concerning this group of disabled people in 1991. These accommodations are not included in the institution category, as they do not have a full-time service offer. , and residents own or pay rent for housing. Some of them have nursing and care services during certain parts of the day. The other dwellings do not have this offer and the residents are rather users of home services in the same way as other ordinary users. Many of these apartments are suitable for residents with functional disabilities and some of them also have some common floor and cleaning.
